The case management department at JCCM provides support to clients to help promote
wellness and autonomy through advocacy, communication, education, identification of resources, and facilitation of services.
Case managers work with clients to develop individual plans to empower them to set
and achieve goals that will lead to greater self sufficiency. Some areas that Case
Managers may help with include obtaining birth certificates, state identification, housing applications, and social security.
They can also make referrals for health care, mental health care, and treatment for substance and alcohol use disorders.

9am to noon to provide basic information to our Intake department and make an
appointment with a case manager.
